		INSTRUCTIONS FOR COMPLETING THE SECTION 8 WAITING LIST APPLICATION Complete all boxes outlined in red. ALL INFORMATION IS REQURIED. Enter the following information in the boxes. County: List the county you currently reside in. Mailing Address: List your current mailing address. City: List the city name you currently reside in. State: List the state you currently reside in. Zip: List the zip code of the city you currently reside in. Phone: List your home telephone number. Work: List your work telephone number if applicable. Message Phone: List any message telephone numbers you may have. Last Name: List the last name of each household member. First Name: List the first name of each household member. Mi Int: List the middle initial of each household member. Social Security Number: List the Social Security number of each household member. Sex: List the sex of each household member. Birth Date: List the date of birth for each household member in MM/DD/YY format Birth City, State: List the city and state of birth for each household member. Relation to HOH: List the relationship of each household member to the head of household; example, Spouse, co-head, child, sibling, etc. Race: List the race of each household member; example, White, Black, American Indian, Asian, Pacific Islander, etc. Income Source: List the source of income for each household member; example, wages, SS (Social Security), SSI (Supplemental Security Income), TANF (Temporary Assistance for Needy Families) etc. Annual Amount: List the approximate annual amount of income from the income source; example, Wages if paid weekly x 52, monthly amounts x 12, etc. Special Accom (Y/N): If a family member with a disability requires any special accommodation to be provided full access to the Housing Agency’s programs and services, indicate by entering Y for yes. Otherwise enter N for no. Once you have completed the application, print it out, sign and date it, and bring it with you to the East Missouri Action Agency Housing Office. YOU MUST BRING THE APPLICATION TO THE HOUSING OFFICE IN PERSON. APPLICATIONS WILL BE ACCEPTED BEGINNING 8:00 a.m. SEPTEMBER 22, 2009. NO APPLICATIONS WILL BE ACCEPTED PRIOR TO THIS DATE AND TIME. The Housing Agency staff will enter the date and time on the application when it is received, sign and date it, and give you a copy for your records. The date and time entered by the Housing Agency staff is the official application date and time. If you live in St. Francois, Madison, Iron, or Ste. Genevieve county, you must bring your application to the following address: 107 Industrial Dr. Park Hills, Mo 63601 573-431-5391 If you live in Cape Girardeau, Perry, or Bollinger county, you must bring your application to the following address: 1111 Linden Cape Girardeau, Mo 63701 573-334-3666 Site pages maintained by EMAA community action people.
